A few things that help

For a wrong or broken verdict: the link to the Judgy page (it starts with judgysays.com/o/).
For extension trouble: which browser, and what the popup said.
For account problems: the email you signed up with. Don't send passwords.

Reporting someone? Every post, comment, shelf item and profile has a report option in its menu — that goes straight to us with the context attached, faster than email.
